The short answer
An AI receptionist for dermatologists built on HighLevel Voice AI costs about $0.20 per answered call, or $92 a month for 450 calls averaging 2.5 minutes, phone line included. Roughly 113 of those calls (25%) would come outside office hours, which is where an AI answering service earns its keep. Emergencies, complaints and anything needing judgment should go to a person.
What the AI should handle
Sort medical and cosmetic callers
Ask one question up front, medical concern or cosmetic service, then follow the right booking rules: insurance and referral details for medical visits, treatment interest and deposit for cosmetic consults.
Offer the earliest openings
Search across providers for the first available new patient slot, book it, and add the caller to the cancellation waitlist if they want something sooner.
Take refill requests
Record the patient's name, date of birth, medication and pharmacy, and create a task for the clinical team. The agent confirms receipt only and never says a refill is approved.
Handle routine rescheduling
Move skin checks and follow-ups to open slots with the same dermatologist and confirm by text, keeping the original visit type and length.
What to send to a person
- Biopsy and pathology results
- Refill approvals and medication questions
- Callers worried about a changing spot who want advice
- Prior authorization questions
HighLevel's voice agents can transfer a live call, create a task, or trigger a workflow that texts the owner. Set the handoff rules before the agent goes live.
Cost per call and per month
Assumptions for a typical dermatology practice: 450 calls a month answered by the AI, averaging 2.5 minutes. Phone minutes are billed per call and rounded up, so each call bills 3 phone minutes at $0.01165.
| Setup | AI cost | Phone line | Per month | Per call |
|---|---|---|---|---|
| Pay per use, Standard voice, GPT-4.1 Mini | $76.50 | $15.73 | $92 | $0.20 |
| Pay per use, ElevenLabs voice, GPT-4.1 Mini | $99.00 | $15.73 | $115 | $0.25 |
| Pay per use, speech-to-speech (OpenAI) | $225.00 | $15.73 | $241 | $0.53 |
| AI Employee Unlimited, $97 flat | $97.00 | $15.73 | $113 | $0.25 |

Compared with staff time
Those 450 calls take about 19 hours of talk time a month. At the US median receptionist wage of $18.27 an hour (May 2025, Bureau of Labor Statistics), that talk time alone is worth about $343, before hold time, breaks, benefits and the calls that come in after hours. An AI receptionist does not replace a good front desk; it catches the overflow and the evenings, and frees staff for the people in front of them.
Sample call
AI AGENTThank you for calling Clearview Dermatology. I'm the practice's AI assistant. Are you calling about a medical concern or a cosmetic service?
CALLERMedical. I have a spot on my arm I want someone to look at.
AI AGENTI can book a new patient visit. The first opening is October 14 at 9:20 a.m. with Dr. Kim. Would you like it, and should I add you to the waitlist for anything sooner?
CALLERYes to both.
AI AGENTDone. Which insurance do you have, and does your plan require a referral?
CALLERBlue Cross PPO. I don't think I need one.
AI AGENTThanks. The team will confirm your benefits before the visit, and if an earlier slot opens you'll get a text to claim it.
CALLERGreat, thank you.
Prompt tips
- Tip 1Open every call with the medical or cosmetic question, and give each branch its own booking rules, calendars and knowledge base section.
- Tip 2State plainly that the agent never comments on how a spot, rash or mole sounds, and always books or routes instead.
- Tip 3List the calls that must reach a person right away, such as biopsy results, with the transfer number for each.
Rules to watch
Results stay with clinicians. Biopsy results and anything tied to a diagnosis are protected health information. Keep them out of the AI's knowledge base and scripts, and route result calls to clinical staff. As a covered entity, the practice needs a business associate agreement with vendors holding patient data; HighLevel's HIPAA package costs $297 a month.
Email and text with patients. HHS guidance says providers may email patients if they apply reasonable safeguards, such as limiting what goes into unencrypted messages, and should honor reasonable requests for other channels. Apply the same caution to texts: time and place, not conditions or procedures.
Good faith estimates for self-pay visits. Under the No Surprises Act, uninsured or self-pay patients usually must get a good faith estimate when a service is scheduled at least three business days ahead or on request. Patients paying cash for cosmetic services may fall into this group, so confirm the process with a compliance adviser.
