The short answer
GoHighLevel for home health care agencies works as one system for calls, texts, bookings, reviews and follow-up. A single home health agency fits on Starter at $97 a month, plus the $297 HIPAA add-on. With our assumed volumes the all-in bill is about $483 a month. An agency running ten home health care agencies on Unlimited pays about $118 per business before HIPAA, or $148 with it.
Home health agencies take calls from three groups at once: families arranging care for a parent, referral sources such as hospital discharge planners, and caregivers calling out of a shift. A missed referral call can send the patient to another agency, and a missed call-out can leave a client without a visit that day. HighLevel can run a referral pipeline, follow up with families who are still deciding, and alert schedulers the moment a caregiver cancels. A Voice AI agent can answer intake questions, take referral details and log call-outs overnight, with clinical and admission decisions left to staff.
Why home health care agencies miss leads
These are the calls and messages a typical home health agency has to catch. Each one is a job, a booking or a sale if someone answers quickly:
- Care for an aging parent at home
- Hospital discharge referral
- Does Medicare or insurance pay for home care
- Caregiver calling out of a shift
- Change or add visit times
- Caregiver job openings
- Billing question from a family
Five workflows to build first
- Referral intake pipelineReferrals from discharge planners and physicians enter a pipeline with stages for received, insurance verified, assessment scheduled and admitted. Each stage has an owner, and any referral untouched for two hours alerts the intake coordinator.
- Family inquiry follow-upFamilies who called but have not started service get a text the next day, an email explaining the assessment process, and a coordinator task on day five. The sequence ends once an assessment is scheduled.
- Caregiver call-out alertWhen a caregiver reports a call-out by phone or a text keyword, a workflow alerts the on-call scheduler at once and logs the shift, so the client can be covered or told in time.
- Referral source thank-youWhen a referral is admitted, send the referring planner a short thank-you with no patient details and tag their contact, so the agency can see which facilities send it the most referrals.
- Caregiver recruitingApplicants from job ads and the careers page enter their own pipeline with automated interview booking and reminders, which keeps recruiting messages out of the client intake inbox.
HighLevel's workflow builder runs all of these, and missed call text back is usually the quickest win.

Adding an AI receptionist
Voice AI can answer the calls above around the clock, book them into the calendar and pass the hard ones to a person. On our assumptions it costs about $0.32 per answered call, phone line included. What the AI should handle, what it should hand to staff and a sample call are in our guide to an AI receptionist for home health care agencies.
Rules to watch
Families and personal representatives. HHS says a person with legal authority to make health care decisions for a patient, such as a guardian or someone holding a health care power of attorney, is generally treated as the patient under the HIPAA Privacy Rule. Other relatives are not automatically. Have the AI take messages rather than discuss an existing client's care.
HIPAA and the HighLevel add-on. Home health agencies that bill Medicare or insurers electronically are HIPAA covered entities. If client names, visit times and care notes go into HighLevel, plan for its $297 a month HIPAA package with a business associate agreement, and keep care plans in the clinical system.
Outbound AI calls need consent. The FCC ruled in February 2024 that AI-generated voices are artificial under the TCPA, so the consent rules for prerecorded calls apply. Get consent that covers AI calls before using outbound Voice AI for visit confirmations or family check-ins.

Which plan
One home health agency fits on Starter at $97 a month, which includes 3 sub-accounts. Move to Unlimited at $297 if you run more than three locations or brands. Agencies serving many home health care agencies should compare Unlimited with Agency Pro and SaaS Mode, which lets you resell and mark up usage.
