The short answer
An AI receptionist for dentists built on HighLevel Voice AI costs about $0.20 per answered call, or $82 a month for 400 calls averaging 2.5 minutes, phone line included. Roughly 120 of those calls (30%) would come outside office hours, which is where an AI answering service earns its keep. Emergencies, complaints and anything needing judgment should go to a person.
What the AI should handle
Book hygiene and new patient exams
Offer open slots from the hygiene calendar, collect name, date of birth, phone and insurance carrier, and book during the call. New patients go into the longer appointment type if the practice schedules them separately.
Capture after-hours pain calls
Take the caller's details and a short description, give the on-call number the dentists approve, and flag the contact for the morning huddle. The agent does not assess symptoms or suggest treatment.
Answer insurance and payment questions
Read from the knowledge base which PPO networks the practice is in and which payment options it offers, then explain that coverage for a specific procedure needs a benefits check by the front desk.
Reschedule and cancel
Find the patient's upcoming appointment, move it to an open slot, and log the change on the contact. If a cancellation frees a prime slot, a workflow can text patients on a short-notice list.
What to send to a person
- Swelling, bleeding or trauma after hours that needs the on-call dentist
- Treatment plan and fee questions tied to a diagnosis
- Insurance breakdowns and claim disputes
- Upset patients or complaints about a past visit
HighLevel's voice agents can transfer a live call, create a task, or trigger a workflow that texts the owner. Set the handoff rules before the agent goes live.
Cost per call and per month
Assumptions for a typical dental practice: 400 calls a month answered by the AI, averaging 2.5 minutes. Phone minutes are billed per call and rounded up, so each call bills 3 phone minutes at $0.01165.
| Setup | AI cost | Phone line | Per month | Per call |
|---|---|---|---|---|
| Pay per use, Standard voice, GPT-4.1 Mini | $68.00 | $13.98 | $82 | $0.20 |
| Pay per use, ElevenLabs voice, GPT-4.1 Mini | $88.00 | $13.98 | $102 | $0.25 |
| Pay per use, speech-to-speech (OpenAI) | $200.00 | $13.98 | $214 | $0.53 |
| AI Employee Unlimited, $97 flat | $97.00 | $13.98 | $111 | $0.28 |

Compared with staff time
Those 400 calls take about 17 hours of talk time a month. At the US median receptionist wage of $18.27 an hour (May 2025, Bureau of Labor Statistics), that talk time alone is worth about $305, before hold time, breaks, benefits and the calls that come in after hours. An AI receptionist does not replace a good front desk; it catches the overflow and the evenings, and frees staff for the people in front of them.

Prompt tips
- Tip 1List every appointment type with its length and the calendar it books into, so the agent never puts a crown prep into a 30 minute hygiene slot.
- Tip 2Write the after-hours pain protocol in the dentists' own words: what to ask, when to give the on-call number, and the wording for anyone describing a medical emergency, including when to call 911.
- Tip 3Paste the accepted insurance list into the knowledge base and tell the agent that coverage for any specific procedure always needs a benefits check.
Rules to watch
HIPAA and the $297 add-on. A dental practice that bills insurance electronically is a HIPAA covered entity, and a vendor holding its patient data needs a business associate agreement. HighLevel accounts are not HIPAA compliant by default; its HIPAA package costs $297 a month, includes a BAA and cannot be canceled once enabled. Have a compliance adviser review the setup.
Replying to online reviews. In 2022 a California dental practice paid $23,000 to settle an HHS investigation into patient information disclosed in responses to online reviews. Keep public replies generic, never confirm that a reviewer is a patient, and switch off automated replies that could mention treatment.
Voicemail and reminder content. HHS allows providers to leave appointment reminders on answering machines and with family members, but advises limiting the details, for example to the practice name, number and what is needed to confirm. Apply the same limit to AI voicemails and reminder texts.
