The short answer
An AI receptionist for massage therapists built on HighLevel Voice AI costs about $0.16 per answered call, or $32 a month for 200 calls averaging 2 minutes, phone line included. Roughly 80 of those calls (40%) would come outside office hours, which is where an AI answering service earns its keep. Emergencies, complaints and anything needing judgment should go to a person.
What the AI should handle
Book sessions
Offer open slots by session length and therapist, collect name, phone and preferences such as pressure or therapist gender, and text a confirmation that includes the cancellation policy.
Sell gift cards
Explain gift card amounts and text a purchase link during the call. The agent never takes card numbers by voice.
Answer pricing and policy questions
Read session prices, add-ons, membership terms and the cancellation policy from the knowledge base, word for word where the policy is concerned.
Book couples sessions
Check for two therapists and a shared room at the same time, book both, and confirm the details to the caller by text.
What to send to a person
- Health questions, injuries or pregnancy considerations
- Complaints about a session or therapist conduct
- Refunds and policy exceptions
- Insurance or auto injury billing
HighLevel's voice agents can transfer a live call, create a task, or trigger a workflow that texts the owner. Set the handoff rules before the agent goes live.
Cost per call and per month
Assumptions for a typical massage practice: 200 calls a month answered by the AI, averaging 2 minutes. Phone minutes are billed per call and rounded up, so each call bills 2 phone minutes at $0.01165.
| Setup | AI cost | Phone line | Per month | Per call |
|---|---|---|---|---|
| Pay per use, Standard voice, GPT-4.1 Mini | $27.20 | $4.66 | $32 | $0.16 |
| Pay per use, ElevenLabs voice, GPT-4.1 Mini | $35.20 | $4.66 | $40 | $0.20 |
| Pay per use, speech-to-speech (OpenAI) | $80.00 | $4.66 | $85 | $0.42 |
| AI Employee Unlimited, $97 flat | $97.00 | $4.66 | $102 | $0.51 |

Compared with staff time
Those 200 calls take about 7 hours of talk time a month. At the US median receptionist wage of $18.27 an hour (May 2025, Bureau of Labor Statistics), that talk time alone is worth about $122, before hold time, breaks, benefits and the calls that come in after hours. An AI receptionist does not replace a good front desk; it catches the overflow and the evenings, and frees staff for the people in front of them.

AI AGENTHi, thanks for calling Riverstone Massage. I'm the AI assistant while our therapists are with clients. How can I help?
CALLERDo you have anything tonight for a 90 minute massage?
AI AGENTYes. Elena has 7:00 p.m. and Jordan has 7:30 p.m. for 90 minutes. Do you have a preference?
CALLERElena at 7.
AI AGENTGreat. Is there a pressure preference or anything Elena should know before the session?
CALLERMedium pressure, and my left shoulder is sore.
AI AGENTI've noted that for Elena. You're booked tonight at 7:00. Our cancellation window is 24 hours, and I'm texting your confirmation and a card-on-file link now.
CALLERPerfect, thanks.
Prompt tips
- Tip 1Store session lengths, prices and which therapists offer each modality in the knowledge base, so the agent never books a prenatal session with someone who does not offer it.
- Tip 2Write the cancellation policy exactly as it should be said and tell the agent to state it on every booking.
- Tip 3Tell the agent that any health concern is noted for the therapist, never answered.
Rules to watch
HIPAA usually does not apply. HIPAA covers health care providers only if they transmit health information electronically for standard transactions such as insurance claims. A cash-only massage practice is usually outside it, so HighLevel's $297 HIPAA add-on is not needed. A practice that bills insurance, for example for injury cases, should get advice.
Consent for texts. FCC rules require written consent for commercial texts such as promotions, while informational texts like booking confirmations may rely on oral consent. Add a clear opt-in for promotional texts to online booking and intake forms.
Review incentives. The FTC's consumer reviews rule bans fake reviews and incentives conditioned on positive sentiment. Google goes further and prohibits any incentive for reviews, so a free upgrade in exchange for a review breaks its policy.
