The short answer
An AI receptionist for med spas built on HighLevel Voice AI costs about $0.20 per answered call, or $72 a month for 350 calls averaging 2.5 minutes, phone line included. Roughly 123 of those calls (35%) would come outside office hours, which is where an AI answering service earns its keep. Emergencies, complaints and anything needing judgment should go to a person.
What the AI should handle
Book consultations
Collect name, phone, the treatment of interest and preferred provider, then book the consult into the right calendar and text the deposit link and intake form.
Quote starting prices from the menu
Read the spa's published price list from the knowledge base, such as a per-unit or per-session starting price, and explain that the provider confirms the plan and final price at the consult.
Sell gift cards and memberships
Explain membership tiers and what each includes, then text a payment link for a gift card or membership signup instead of taking card numbers by voice.
Take post-treatment messages
Log the client's question, mark it clinical, and alert the nurse on duty by text. The agent tells the caller when to expect a callback and reads the emergency wording the medical director approved.
What to send to a person
- Questions about side effects, reactions or whether a treatment suits someone
- Medical history and screening before treatment
- Refunds and complaints
- Custom treatment plans and package pricing
HighLevel's voice agents can transfer a live call, create a task, or trigger a workflow that texts the owner. Set the handoff rules before the agent goes live.
Cost per call and per month
Assumptions for a typical med spa: 350 calls a month answered by the AI, averaging 2.5 minutes. Phone minutes are billed per call and rounded up, so each call bills 3 phone minutes at $0.01165.
| Setup | AI cost | Phone line | Per month | Per call |
|---|---|---|---|---|
| Pay per use, Standard voice, GPT-4.1 Mini | $59.50 | $12.23 | $72 | $0.20 |
| Pay per use, ElevenLabs voice, GPT-4.1 Mini | $77.00 | $12.23 | $89 | $0.25 |
| Pay per use, speech-to-speech (OpenAI) | $175.00 | $12.23 | $187 | $0.53 |
| AI Employee Unlimited, $97 flat | $97.00 | $12.23 | $109 | $0.31 |
The breakdown of every Voice AI charge is in our Voice AI pricing guide. Agencies can price this as a monthly service with the calculator's Price a client tab, preloaded for a med spa.
Compared with staff time
Those 350 calls take about 15 hours of talk time a month. At the US median receptionist wage of $18.27 an hour (May 2025, Bureau of Labor Statistics), that talk time alone is worth about $266, before hold time, breaks, benefits and the calls that come in after hours. An AI receptionist does not replace a good front desk; it catches the overflow and the evenings, and frees staff for the people in front of them.

AI AGENTHi, thanks for calling Glow Aesthetics. I'm the spa's AI assistant. How can I help tonight?
CALLERHow much is Botox with you?
AI AGENTBotox is priced per unit, and the starting price is on our menu. Your provider confirms how many units you need at a consult. I can text you the menu and book a consult if you like.
CALLERSure. What do you have Saturday?
AI AGENTMaya, our nurse injector, has Saturday at 10:00 a.m. or 1:30 p.m. Which would you like?
CALLER10 a.m.
AI AGENTBooked. Consults need a deposit that goes toward your treatment. I'm texting the deposit link, our menu and the intake form now.
CALLERGreat, thanks.
Prompt tips
- Tip 1Load the full service menu with starting prices and which provider types perform each treatment, so the agent books injectables only with injectors.
- Tip 2Add a rule that any question about side effects, reactions or whether a treatment is right for someone ends in a message to the nurse, never an answer.
- Tip 3Write the deposit and cancellation policy word for word, since callers ask about it and the agent should repeat it exactly.
Rules to watch
HIPAA depends on how the spa operates. HIPAA applies to health care providers that transmit health information electronically for standard transactions, such as insurance claims. A cash-only med spa may fall outside it, but treatment notes, photos and medical histories are sensitive either way. Confirm status with a healthcare attorney before deciding against HighLevel's $297 a month HIPAA package.
Claims about results. The FTC expects health-related advertising claims to be backed by competent and reliable scientific evidence, and says anyone taking part in deceptive marketing can be liable. Keep promised results out of the AI's scripts and follow-up texts, and let providers discuss outcomes in person.
Reviews and incentives. The FTC's consumer reviews rule, effective October 21, 2024, bans fake reviews and incentives conditioned on positive sentiment. Google goes further and prohibits offering any incentive for reviews. Do not attach discounts or free add-ons to review requests.
