The short answer
GoHighLevel for med spas works as one system for calls, texts, bookings, reviews and follow-up. A single med spa fits on Starter at $97 a month, plus the $297 HIPAA add-on. With our assumed volumes the all-in bill is about $483 a month. An agency running ten med spas on Unlimited pays about $119 per business before HIPAA, or $149 with it.
Med spas sell treatments people research on their phones at night, so inquiries often arrive by call, Instagram DM and web form after the front desk has gone home. Callers want injectable prices, a consult this week, and details on memberships or packages, and a slow reply can send them to whichever spa is running the next ad. HighLevel gives a med spa one inbox for calls, texts and social messages, booking with deposits, membership billing and rebooking reminders timed to each treatment. Voice AI can answer after hours and book consults, while the nurse or provider keeps every medical conversation.
Why med spas miss leads
These are the calls and messages a typical med spa has to catch. Each one is a job, a booking or a sale if someone answers quickly:
- Price of Botox or filler
- Book a consultation
- Membership and package questions
- Laser hair removal availability
- Reschedule a treatment
- Question for the nurse after treatment
- Buy a gift card
Five workflows to build first
- Treatment-timed rebookingTag each client by the treatment they had and send a rebooking text at the interval the provider sets for that treatment. The link opens the right service with the right provider, so the client books in two taps.
- Consult deposit and remindersCollect a deposit through a paid calendar or payment link when a consult is booked, then send reminders two days and two hours before. The confirmation text spells out the deposit and no-show policy.
- Instagram and Facebook DM routingMessages from Instagram and Facebook land in the unified inbox beside calls and texts. A workflow tags pricing questions and assigns them to the coordinator, so DMs stop sitting unread in someone's phone.
- Membership failed paymentWhen a membership charge fails, text the client a link to update their card and create a task if it is still unpaid after three days. Pause member pricing on new bookings until it is resolved.
- Review request after the follow-upSend the review request after the follow-up visit, to every client who completed it. The FTC bans incentives conditioned on positive reviews, and Google prohibits review incentives entirely, so no credit for a review.
HighLevel's workflow builder runs all of these, and missed call text back is usually the quickest win.

Adding an AI receptionist
Voice AI can answer the calls above around the clock, book them into the calendar and pass the hard ones to a person. On our assumptions it costs about $0.20 per answered call, phone line included. What the AI should handle, what it should hand to staff and a sample call are in our guide to an AI receptionist for med spas.
Rules to watch
HIPAA depends on how the spa operates. HIPAA applies to health care providers that transmit health information electronically for standard transactions, such as insurance claims. A cash-only med spa may fall outside it, but treatment notes, photos and medical histories are sensitive either way. Confirm status with a healthcare attorney before deciding against HighLevel's $297 a month HIPAA package.
Claims about results. The FTC expects health-related advertising claims to be backed by competent and reliable scientific evidence, and says anyone taking part in deceptive marketing can be liable. Keep promised results out of the AI's scripts and follow-up texts, and let providers discuss outcomes in person.
Reviews and incentives. The FTC's consumer reviews rule, effective October 21, 2024, bans fake reviews and incentives conditioned on positive sentiment. Google goes further and prohibits offering any incentive for reviews. Do not attach discounts or free add-ons to review requests.

Which plan
One med spa fits on Starter at $97 a month, which includes 3 sub-accounts. Move to Unlimited at $297 if you run more than three locations or brands. Agencies serving many med spas should compare Unlimited with Agency Pro and SaaS Mode, which lets you resell and mark up usage.
