The short answer
GoHighLevel for mental health therapists works as one system for calls, texts, bookings, reviews and follow-up. A single therapy practice fits on Starter at $97 a month, plus the $297 HIPAA add-on. With our assumed volumes the all-in bill is about $439 a month. An agency running ten mental health therapists on Unlimited pays about $75 per business before HIPAA, or $104 with it.
People often call a therapy practice at a hard moment, frequently in the evening, and a full voicemail box or a slow callback can mean they move on. Solo clinicians and small group practices are in session most of the day, so inquiries wait hours for a reply. The rest of the phone load is logistics: which therapist fits, insurance, session fees and the cancellation policy. HighLevel can handle inquiry intake, consultation booking, reminders and waitlists. A Voice AI agent can answer routine questions and book consultations, but it must recognize a crisis call and point the caller to 988 or 911 immediately.
Why mental health therapists miss leads
These are the calls and messages a typical therapy practice has to catch. Each one is a job, a booking or a sale if someone answers quickly:
- Is a therapist accepting new clients
- Do you take my insurance
- Fee for a self-pay session
- Book a free phone consultation
- Therapy for my teenager
- Telehealth or in-person options
- Reschedule or cancel a session
Five workflows to build first
- Inquiry to consultationNew inquiries from calls, web forms or a directory profile get a same-day text with a link to book a short phone consultation, then one gentle reminder the next day. Staff see every inquiry in a pipeline sorted by clinician fit.
- Waitlist by clinicianWhen no clinician with the right specialty has openings, add the inquiry to a waitlist tagged by specialty, insurance and format, and text them when a matching slot opens. Remove them once they book.
- Reminders with minimal detailSend reminders with the day, the time and whether the session is in person or telehealth, and nothing about the reason for care. Include the cancellation policy in the first confirmation text.
- Good faith estimate taskWhen a self-pay client books, create a task for staff to send the good faith estimate the No Surprises Act calls for, and store a copy on the client's record.
- Referral out when fullIf the practice cannot help, a workflow texts a short list of referral resources the clinical director approves, so no inquiry ends without a next step.
HighLevel's workflow builder runs all of these, and missed call text back is usually the quickest win.

Adding an AI receptionist
Voice AI can answer the calls above around the clock, book them into the calendar and pass the hard ones to a person. On our assumptions it costs about $0.24 per answered call, phone line included. What the AI should handle, what it should hand to staff and a sample call are in our guide to an AI receptionist for mental health therapists.
Rules to watch
Keep clinical records out of HighLevel. HHS notes that records from federally assisted substance use disorder programs carry protections under 42 CFR Part 2 on top of HIPAA. Keep session notes in the EHR and use HighLevel only for scheduling and outreach, with the $297 a month HIPAA package and its business associate agreement in place.
Crisis calls. The 988 Suicide and Crisis Lifeline is available 24 hours a day by call, text or chat. Build the AI's crisis path first: recognize risk language, give 988 and 911, and transfer to an on-call clinician where one exists. Have a licensed clinician review the script.
Good faith estimates. Under the No Surprises Act, uninsured and self-pay clients usually must receive a good faith estimate when a service is scheduled at least three business days ahead or when they ask. Self-pay therapy clients fall into this group, so build the estimate into booking.

Which plan
One therapy practice fits on Starter at $97 a month, which includes 3 sub-accounts. Move to Unlimited at $297 if you run more than three locations or brands. Agencies serving many mental health therapists should compare Unlimited with Agency Pro and SaaS Mode, which lets you resell and mark up usage.
