The short answer
An AI receptionist for orthodontists built on HighLevel Voice AI costs about $0.24 per answered call, or $60 a month for 250 calls averaging 3 minutes, phone line included. Roughly 75 of those calls (30%) would come outside office hours, which is where an AI answering service earns its keep. Emergencies, complaints and anything needing judgment should go to a person.
What the AI should handle
Book new patient consultations
Collect the patient's age, the parent's name and phone, the referring dentist, and whether the family is asking about braces or aligners, then book a consult slot and text the intake form link.
Handle broken bracket and wire calls
Take the details, book the next repair slot, and text any comfort instructions the orthodontist has written. Escalate to the on-call line when the parent says it cannot wait.
Explain payment plan basics
Read the practice's standard terms from the knowledge base, such as whether monthly plans and down payment options exist, and tell parents the exact fee comes from the consult. No quotes for a specific child.
Reschedule adjustment visits
Find the patient's next adjustment, offer open slots after school, and confirm the change by text to the parent. Flag patients who have moved an appointment more than twice for staff follow-up.
What to send to a person
- Treatment fee quotes and contract questions
- Benefit checks for orthodontic coverage
- Custody or consent questions about who can decide for a minor
- Calls about injuries to the mouth or face
HighLevel's voice agents can transfer a live call, create a task, or trigger a workflow that texts the owner. Set the handoff rules before the agent goes live.
Cost per call and per month
Assumptions for a typical orthodontic practice: 250 calls a month answered by the AI, averaging 3 minutes. Phone minutes are billed per call and rounded up, so each call bills 3 phone minutes at $0.01165.
| Setup | AI cost | Phone line | Per month | Per call |
|---|---|---|---|---|
| Pay per use, Standard voice, GPT-4.1 Mini | $51.00 | $8.74 | $60 | $0.24 |
| Pay per use, ElevenLabs voice, GPT-4.1 Mini | $66.00 | $8.74 | $75 | $0.30 |
| Pay per use, speech-to-speech (OpenAI) | $150.00 | $8.74 | $159 | $0.63 |
| AI Employee Unlimited, $97 flat | $97.00 | $8.74 | $106 | $0.42 |

Compared with staff time
Those 250 calls take about 13 hours of talk time a month. At the US median receptionist wage of $18.27 an hour (May 2025, Bureau of Labor Statistics), that talk time alone is worth about $228, before hold time, breaks, benefits and the calls that come in after hours. An AI receptionist does not replace a good front desk; it catches the overflow and the evenings, and frees staff for the people in front of them.
Sample call
AI AGENTHi, you've reached Bright Path Orthodontics. I'm the office's AI assistant. How can I help?
CALLERMy daughter's dentist said she might need braces. How do we get started?
AI AGENTWe start with a consultation with Dr. Nguyen. How old is your daughter, and which dentist referred you?
CALLERShe's twelve. Dr. Morales at Riverside Family Dental.
AI AGENTThank you. I have Tuesday at 4:00 p.m. or Wednesday at 3:30 p.m. Would either work after school?
CALLERWednesday. Roughly what do braces cost?
AI AGENTThe fee depends on her treatment plan, which Dr. Nguyen explains at the consult along with monthly payment options. I've booked Wednesday at 3:30 and I'm texting you the new patient forms.
CALLERPerfect, thank you.
Prompt tips
- Tip 1Tell the agent to ask whether the caller is the patient or a parent, and to store the parent as the contact for anyone under 18.
- Tip 2Give it one fixed sentence for fee questions that names the consult as the place for pricing, so it never guesses a treatment cost.
- Tip 3Separate routine repairs from urgent calls: define what counts as a broken bracket visit and what goes straight to the on-call line.
Rules to watch
Parents as personal representatives. HHS says a parent is usually the personal representative of a minor child under HIPAA and can exercise the child's privacy rights, with exceptions tied to state law, court orders and certain agreements. Record which adults may discuss each patient's care, and send custody or consent questions to staff.
HIPAA add-on for patient records. An orthodontic practice that bills insurance electronically is a HIPAA covered entity. If HighLevel will hold treatment details, intake answers or appointment histories, plan for HighLevel's $297 a month HIPAA package with its business associate agreement, and get a compliance adviser to review the setup.
Consent for texts. FCC rules require written consent for commercial texts, while informational texts such as appointment reminders may rely on oral consent. Put a clear opt-in on consult forms before sending promotional messages like referral contests or aligner specials.
