The short answer
An AI receptionist for weight loss clinics built on HighLevel Voice AI costs about $0.24 per answered call, or $60 a month for 250 calls averaging 3 minutes, phone line included. Roughly 88 of those calls (35%) would come outside office hours, which is where an AI answering service earns its keep. Emergencies, complaints and anything needing judgment should go to a person.
What the AI should handle
Explain program options and prices
Read program tiers, what each includes and the monthly price from the knowledge base, and say that a provider decides which program and any medication fits at the consult.
Book consultations
Collect name, phone, email, in-person or telehealth preference, and insurance or self-pay, then book the consult and text the intake form.
Handle reschedules
Move a weigh-in or check-in to another open slot, confirm by text, and keep the patient's program schedule intact.
Take membership changes
Log requests to pause or cancel, restate the clinic's written policy, and create a task for staff to process it. The agent never issues refunds.
What to send to a person
- Who qualifies for any medication or medical program
- Side effects or health concerns
- Refunds and billing disputes
- Insurance prior authorizations
HighLevel's voice agents can transfer a live call, create a task, or trigger a workflow that texts the owner. Set the handoff rules before the agent goes live.
Cost per call and per month
Assumptions for a typical weight loss clinic: 250 calls a month answered by the AI, averaging 3 minutes. Phone minutes are billed per call and rounded up, so each call bills 3 phone minutes at $0.01165.
| Setup | AI cost | Phone line | Per month | Per call |
|---|---|---|---|---|
| Pay per use, Standard voice, GPT-4.1 Mini | $51.00 | $8.74 | $60 | $0.24 |
| Pay per use, ElevenLabs voice, GPT-4.1 Mini | $66.00 | $8.74 | $75 | $0.30 |
| Pay per use, speech-to-speech (OpenAI) | $150.00 | $8.74 | $159 | $0.63 |
| AI Employee Unlimited, $97 flat | $97.00 | $8.74 | $106 | $0.42 |

Compared with staff time
Those 250 calls take about 13 hours of talk time a month. At the US median receptionist wage of $18.27 an hour (May 2025, Bureau of Labor Statistics), that talk time alone is worth about $228, before hold time, breaks, benefits and the calls that come in after hours. An AI receptionist does not replace a good front desk; it catches the overflow and the evenings, and frees staff for the people in front of them.

Prompt tips
- Tip 1Write one fixed sentence for every medication question, saying the provider decides eligibility at the consult, and forbid the agent from adding anything to it.
- Tip 2Put program prices, what each tier includes and the cancellation policy in the knowledge base as plain numbers and rules, so the agent quotes them exactly.
- Tip 3Tell the agent never to mention a weight, a result or a timeline, even when a caller asks what is typical.
Rules to watch
Claims about weight loss. The FTC expects health-related advertising claims to be backed by competent and reliable scientific evidence, and says anyone taking part in deceptive marketing can be liable. Keep promised pounds, timelines and outcomes out of the AI's scripts and follow-up messages.
HIPAA and medication programs. Clinics that bill insurance electronically are HIPAA covered entities, and weights and medications are sensitive in any case. If they go into HighLevel, budget for its $297 a month HIPAA package with a business associate agreement, and get a compliance review before launch.
Testimonials and reviews. The FTC's consumer reviews rule, effective October 21, 2024, bans fake reviews and incentives tied to positive sentiment. If the clinic is a HIPAA covered entity, HHS guidance means patient success stories used in marketing generally need the patient's written authorization.
