The short answer
An AI receptionist for chiropractors built on HighLevel Voice AI costs about $0.16 per answered call, or $48 a month for 300 calls averaging 2 minutes, phone line included. Roughly 90 of those calls (30%) would come outside office hours, which is where an AI answering service earns its keep. Emergencies, complaints and anything needing judgment should go to a person.
What the AI should handle
Book new patient exams
Collect name, phone, how they heard about the clinic, and whether the visit relates to an auto accident or work injury, then book a new patient slot and text the intake forms.
Move care plan visits
Find the patient's next visit, offer the nearest open times with the same doctor, and confirm by text. Offer slots in the same week first so the visit frequency on the plan holds.
Answer pricing and coverage basics
Give the cash visit price and payment options from the knowledge base, and explain that insurance and Medicare coverage depend on the plan, so staff verify benefits before the first visit.
Route auto accident callers
Tag callers injured in a car accident, collect the claim details the clinic asks for, and create a task for the personal injury coordinator, who handles attorney and insurer paperwork.
What to send to a person
- Benefit verification and Medicare questions
- Personal injury cases involving attorneys or insurers
- Callers describing numbness, weakness or a recent serious injury
- Billing disputes
HighLevel's voice agents can transfer a live call, create a task, or trigger a workflow that texts the owner. Set the handoff rules before the agent goes live.
Cost per call and per month
Assumptions for a typical chiropractic clinic: 300 calls a month answered by the AI, averaging 2 minutes. Phone minutes are billed per call and rounded up, so each call bills 2 phone minutes at $0.01165.
| Setup | AI cost | Phone line | Per month | Per call |
|---|---|---|---|---|
| Pay per use, Standard voice, GPT-4.1 Mini | $40.80 | $6.99 | $48 | $0.16 |
| Pay per use, ElevenLabs voice, GPT-4.1 Mini | $52.80 | $6.99 | $60 | $0.20 |
| Pay per use, speech-to-speech (OpenAI) | $120.00 | $6.99 | $127 | $0.42 |
| AI Employee Unlimited, $97 flat | $97.00 | $6.99 | $104 | $0.35 |

Compared with staff time
Those 300 calls take about 10 hours of talk time a month. At the US median receptionist wage of $18.27 an hour (May 2025, Bureau of Labor Statistics), that talk time alone is worth about $183, before hold time, breaks, benefits and the calls that come in after hours. An AI receptionist does not replace a good front desk; it catches the overflow and the evenings, and frees staff for the people in front of them.

Prompt tips
- Tip 1Give the agent each visit type with its length, new patient exam versus adjustment, so it never books a first visit into a 15 minute slot.
- Tip 2Add a hard rule that the agent does not say whether Medicare or a plan covers care, and always hands benefit questions to staff.
- Tip 3Have the doctors list the complaints they want escalated immediately, plus the exact transfer or 911 wording the agent should use for each.
Rules to watch
Medicare coverage is narrow. Medicare Part B covers only adjustments of the spine by a chiropractor to correct a subluxation, and does not cover other services or tests a chiropractor orders, such as X-rays or massage therapy. The AI should never tell a Medicare caller that a visit is covered; staff confirm.
HIPAA and the HighLevel add-on. Chiropractors appear on HHS's list of providers that are covered entities when they transmit health information electronically for standard transactions such as claims. If HighLevel will hold visit histories or intake answers, budget for its $297 a month HIPAA package with a business associate agreement, and have the setup reviewed.
Asking for reviews. Google's review policy prohibits discouraging negative reviews, selectively soliciting positive ones, and offering incentives for reviews. Send the same request to every patient and skip gift card or discount offers tied to a review.
