The short answer
GoHighLevel for physical therapists works as one system for calls, texts, bookings, reviews and follow-up. A single physical therapy clinic fits on Starter at $97 a month, plus the $297 HIPAA add-on. With our assumed volumes the all-in bill is about $471 a month. An agency running ten physical therapists on Unlimited pays about $107 per business before HIPAA, or $137 with it.
Physical therapy clinics depend on patients finishing a plan of care, and the phone is where plans come apart: a patient cancels, nobody reschedules, and the case goes quiet. New patients call with a referral in hand or asking whether they need one at all, and questions about copays and visit limits eat front desk time. HighLevel adds visit reminders, cancellation recovery, discharge follow-up and review requests alongside the clinic's EMR. A Voice AI agent can answer while the desk is checking patients in, book evaluations, and log cancellations before the open slot is lost.
Why physical therapists miss leads
These are the calls and messages a typical physical therapy clinic has to catch. Each one is a job, a booking or a sale if someone answers quickly:
- Book an initial evaluation
- Do I need a referral from my doctor
- Cancel or reschedule a visit
- Copay and visit limit questions
- Workers' comp or auto injury claim
- Hours and closest location
- Send my notes to my surgeon
Five workflows to build first
- Cancellation recoveryWhen a visit is canceled, text the patient the next three open slots with the same therapist. If they have not rebooked within a day, create a front desk task, so the gap does not quietly turn into an early discharge.
- Evaluation welcomeAfter an evaluation is booked, send intake forms, what to wear, where to park, and a reminder to bring the referral or prescription if there is one. Answers save to the contact record before the first visit.
- Visit remindersSend a text the day before each visit with reply options to confirm or reschedule. Replies route to the shared inbox, and every reschedule request creates a task with the patient's therapist named on it.
- Discharge check-inTwo weeks after discharge, text a short check-in asking how the patient is doing and whether they want a follow-up visit or a wellness program. Replies are tagged for the treating therapist to review.
- Review request at dischargeSend the review request when the patient completes their plan, not after the first visit. Send it to every discharged patient, since Google's policy bars selectively soliciting positive reviews.
HighLevel's workflow builder runs all of these, and missed call text back is usually the quickest win.

Adding an AI receptionist
Voice AI can answer the calls above around the clock, book them into the calendar and pass the hard ones to a person. On our assumptions it costs about $0.20 per answered call, phone line included. What the AI should handle, what it should hand to staff and a sample call are in our guide to an AI receptionist for physical therapists.
Rules to watch
Direct access varies by state. APTA reports that all 50 states, the District of Columbia and the U.S. Virgin Islands allow provisional or unrestricted direct access to physical therapists, but some states still attach conditions to treatment without a referral, and insurers set their own rules. The AI should not tell callers a referral is unnecessary.
HIPAA and the HighLevel add-on. PT clinics that bill insurance electronically are HIPAA covered entities. If HighLevel stores surgery dates, diagnoses or intake answers, plan for its HIPAA package at $297 a month, which includes a business associate agreement and cannot be canceled once enabled, and have a compliance adviser review it.
Good faith estimates for self-pay patients. Under the No Surprises Act, uninsured or self-pay patients usually must receive a good faith estimate when care is scheduled at least three business days ahead, or when they ask. Make sure cash-pay patients booked by the AI get one from staff.

Software physical therapists often use
WebPT, Raintree Systems, Prompt Health, Net Health, SPRY, Jane. Check the HighLevel marketplace or ask HighLevel before assuming any of these connect directly.
Which plan
One physical therapy clinic fits on Starter at $97 a month, which includes 3 sub-accounts. Move to Unlimited at $297 if you run more than three locations or brands. Agencies serving many physical therapists should compare Unlimited with Agency Pro and SaaS Mode, which lets you resell and mark up usage.
