The short answer
GoHighLevel surveys are multi-slide forms, included in every HighLevel plan, that show questions one screen at a time and can send each person down a different path with Jump To logic. A survey can create the contact as soon as the first slide is done, take payments with order bumps, and pass answers to workflows. Quizzes add overall and category scores, result messages and certificates. Use a survey for lead qualification or client intake; use a plain form when every question fits on one screen.

How surveys differ from forms
Surveys split questions across slides, which lowers the effort of long questionnaires, and they get the one logic action forms do not: Jump To, which moves the respondent to a chosen slide based on an answer. HighLevel adds automatic cycle protection so jump rules cannot trap someone in a loop. Surveys also share the form tools for redirect, custom messages, disqualifying a lead and showing or hiding fields, with rules that use either AND or OR. A progress bar shows on mobile, and multi-step surveys can save progress so a respondent can leave and come back later without losing answers.
Capturing leads who do not finish
The Create Contact on Partial Submission option saves the contact once the first slide is completed and the respondent clicks Next, so a lead who drops off on slide three is still in the CRM. It only works when contact fields such as email or phone sit on the first slide. HighLevel lists two catches: partial creation is not supported when logic skips straight to the last slide, and contact details are not updated when a respondent jumps to the second-to-last slide, only at final submission. Keep contact details up front and avoid early jumps to the end.
Quizzes and scoring
Quizzes are surveys with scoring. They support an overall score and category scores, personality-style results based on the highest or lowest category, custom result pages and messages, certificate or badge issuance, and mapping scores into contact fields. The Quiz Submitted workflow trigger filters on quiz name, overall score, category score or custom score fields, with Between ranges that include both ends. That makes it easy to send a score of 80 and up to a sales call booking while lower scores get a nurture sequence.
Payments, exports and documents
Surveys can sell a main product with one or more order bumps; bumps only appear when a main product is present, and only one product per survey can be recurring. HighLevel's February 2026 provider matrix shows PayPal does not support survey payments, so use Stripe or another supported card processor. Answers can be exported, sent to Google Sheets, or saved as a PDF submission document on the contact, and surveys can be scheduled to open and close on set dates, which suits application windows and event registrations.
